Dr. Waguespack has created some convenient on-line forms to help make your first and subsequent visits to our office an easy and no-hassle experience.  Questionnaires within this website may be printed, completed, and brought to the office.  If you are a new patient for Dr. Waguespack please complete the New Patient Registration Forms by clicking on New Patient Registration Form to the left. 

Also, please take a moment to fill out 1-2 additional questionnaires that best describe the reason for your visit.
